What To Bring For First Visit

• Medical Record from previous doctors and hospitals.
• List of Prescription medications, OTC medications, Vitamins and herbal drugs.
• Write down all the questions and concerns, so you do not forget during history.
• Official Picture ID.
• Insurance Card or cards
• If yours insurance require/assign you to go to specific Primary Care Provider (PCP), than call your insurance and change your PCP to Dr. Tehseen Naqvi or Progressive Medical care.
• If you are participating with Medicaid program, you definitely have to change Primary Care Provider to Dr. Tehseen Naqvi/Progressive Medical Care. We participate with following Medicaid’s insurance.
